They are not interchangeable, and most disappointing results come from using one where the other was needed. Botox is a neuromodulator: it relaxes the muscle producing a movement line. Fillers replace lost volume and rebuild structure.
Lines that appear only when you move — frowning, raising the brows, smiling — are Botox territory. Lines and hollows visible when your face is completely at rest are volume, which means filler. Rough surface texture and sun damage are neither, and respond to resurfacing, peels or microneedling.
Many faces need a combination, in a specific order. Assessment first is how you avoid paying for the wrong treatment.
The product is standardised; the outcome is not. Units, dilution, injection depth and exact placement relative to your own muscle pattern determine whether the result looks rested or frozen, and whether a brow drops.
Common errors we correct: too much in the forehead without addressing the glabella, which drops the brows and makes the eyes look heavy; treating crow's feet aggressively in someone whose lower lid support is already weak; and under-dosing a strong frown muscle so the effect fades in six weeks and the patient concludes Botox does not work on them.
Onset is gradual over several days, with the full effect visible around two weeks. We review at that point and adjust, because the second treatment should be better informed than the first.
Neuromodulators have medical uses beyond wrinkles. Excessive underarm sweating that does not respond to prescription antiperspirants often responds well, with results lasting months.
