Home Visits & Assisted Living Care in Miami-Dade

Primary care and dermatology delivered at the patient's address — for ALF residents and homebound patients, with a bilingual clinical team.

TL;DR

We bring primary care and dermatology to ALF residents and homebound patients across Miami-Dade — billed to Medicare, Medicare Advantage, and most managed Medicaid plans.

  • Two specialties in one visit means fewer transports and fewer outside referrals.
  • Wound and pressure-injury assessment, rashes, skin cancer checks, and biopsy at the bedside.
  • Chronic disease management, medication review, lab draws, and post-hospital follow-up.
  • Bilingual clinicians — English and Spanish.
  • Family, facility staff, or a case manager can request the visit: (305) 676-8217.

Care Delivered at the Patient's Address

For residents of assisted living facilities (ALFs) and for homebound patients, transportation is the single biggest barrier to getting care. Skin conditions go unexamined for months, chronic conditions drift out of control, and small problems become hospital admissions.

Paradise Medical Center brings both sides of the practice to the patient: primary care and dermatology. Dr. Teresa Moreno, DNP, and our bilingual team see patients in ALFs and private residences across Miami-Dade, with the same documentation and follow-up as an in-clinic visit.

Why Facilities Work With Us

Insurance and Eligibility

Home and facility visits are billed to Medicare, Medicare Advantage, and most managed Medicaid plans the same way an office visit is, when the patient meets the criteria for care in the home. We verify eligibility and any prior-authorization requirement before the first visit — see accepted insurance.

Coverage areas include Westchester, West Miami, Flagami, Coral Gables, Sweetwater, Doral, Miami Springs, Hialeah, Kendall, and surrounding Miami-Dade neighborhoods.

How to Arrange a Visit

Family members, facility administrators, and case managers can all start the process. Call Primary Care at (305) 676-8217 or Dermatology at (786) 738-9515, or send a request with “home visit” or “ALF” in the message field.

Tell us the address, the patient's insurance, and the main concern. Our team confirms coverage for the address and schedules the visit — usually within the same week.
