It is not appropriate during pregnancy or breastfeeding, with a personal or family history of medullary thyroid carcinoma or MEN2, or with active pancreatitis.

Week one is the evaluation, labs and the first 0.25 mg dose with injection training. Weeks two and three are about tolerance: how appetite responds, whether nausea appears, and whether protein intake is holding at target. Around week four we decide whether to increase the dose or hold, based on results and side effects rather than a fixed calendar.
Follow-up is weekly or as needed, in person or by phone. We track weight, BMI, waist measurements and photos, because early fat loss often shows in measurements before the scale moves much.
