Tirzepatide is a dual GIP and GLP-1 receptor agonist. It engages two incretin pathways rather than one, which affects appetite, gastric emptying and insulin response together. In practice patients often describe a flatter, quieter appetite than they experienced on a single-pathway medication, with fewer evening cravings.

Adults with a BMI of 30 or above, or 27 and above with a weight-related condition, are typical candidates — particularly patients with prediabetes, type 2 diabetes, metabolic syndrome or a plateau on a maximum semaglutide dose. It is not used in pregnancy or breastfeeding, with a history of medullary thyroid carcinoma or MEN2, or with active pancreatitis.

We start at 2.5 mg and move up only when the current dose is tolerated and results justify it. Escalating too quickly is the most common reason patients quit these medications, and it is avoidable. Follow-up happens weekly or as needed, in person or by phone, with weight, BMI, measurements and photos recorded so progress is visible even in weeks the scale stalls.
Blood pressure, blood sugar and lipids are rechecked as weight comes down, because doses of existing medications for hypertension and diabetes often need adjusting.

Tirzepatide acts on two hormone receptors instead of one and is a costlier medication to source. On average it produces greater appetite suppression and greater total weight loss, which is what the higher weekly price reflects.
Treatment starts at 2.5 mg once weekly and increases in steps, commonly to 5, 7.5, 10, 12.5 and up to 15 mg, only as tolerated. Many patients settle in the middle of that range rather than the top.
Appetite usually changes within the first one to two weeks. Visible weight change typically appears by weeks 8 to 12 and continues over six to twelve months while doses are optimized.
Nausea, constipation or loose stools, reflux, fullness and occasional fatigue, concentrated in the days after a dose increase. Slower titration, smaller meals and hydration resolve most of it. We screen for medullary thyroid cancer history, MEN2, pancreatitis and pregnancy before prescribing.
Yes. If weight loss stalls on a maximum semaglutide dose or appetite control is incomplete, switching is common and we handle the transition dosing in the clinic.
